The cartridge inside wall is splined, then there is a white plastic piece that screws to threads outside of that and then handle screws inside of that white plastic piece to tighten against the tile to create tight seal (I think).Its probably just heavily corroded. You may need to pry it off being careful not to crack your tiles. You might need to put a 2 x 4 or similar against the tile and then gingerly using a pry bar try to remove it. But it still must be attached somehow. And by looking at the pic I can only assume a set screw.
