Stern82 my 989 M2 was doing about the same thing. The magazine seemed a bit sloppy (loose) when inserted. I tried firing the gun while applying upward pressure on magazine and the gun functioned properly. So I slightly bent the magazine release lever forward so the mag was held more firmly and slightly deeper in the mag well.
